Please download the medical form PAGE 1 first. If necessary, you must have a physicians release before you will be allowed to participate in the scuba portions of the course.
Download the entire medical release forms and have your physician issue a medical release for you. This release MUST be presented prior to participating in any water activities.
These documents can be faxed in with your registration papers or delivered the day of the first class.
